Volunteer Waiver

By filling out and submitting this form, you are indicating that you (and any minors present with you) desire to work as a volunteer for Katy Responds, a nonprofit corporation. You knowingly release, absolve, indemnify and hold harmless Katy Responds, its partners, members, trustees, board, leadership, and staff as well as organizers, site-leaders and other workers from loss of any kind related to my activities for Katy Responds.

THIS MAY INCLUDE ANY CLAIMS AGAINST ANY PREVIOUSLY IDENTIFIED PARTIES FOR CLAIMS INCLUDING THOSE OF NEGLIGENCE OR GROSS NEGLIGENCE.

ASSUMPTION OF RISK: You understand that the activities may be hazardous to the volunteer, and, the volunteer expressly and specifically assumes all the risk of injury or harm in these activities. The volunteer has no known physical or mental condition that would impair their capability for full participation as intended or expected of them.

TEXAS LAW: You expressly agree that this release, waiver and indemnity agreement is intended to be as broad and inclusive as permitted by the laws of the State of Texas.

INSURANCE: Each volunteer is expected and encouraged to obtain their own medical or health insurance coverage.

PHOTOGRAPHIC RELEASE: You hereby grant Katy Responds permission to use, at no cost, video tape recordings and/or photographs of you and/or minor children in your care and custody taken while participating in today's activities, in any manner or form or for any lawful purpose at any time.
